Application for SONGA membership

I, the undersigned, hereby apply for the membership of SONGA, in accordance with its constitution and by-law as of now, or as they may hereafter legally altered. The following information on myself are true to the best of my knowledge. I will submit a detailed curriculum-vitae and a photograph (by mail) upon acceptance of my membership.
